Generic Doxepin

Common use
Doxepin is a tricyclic antidepressant used primarily to treat depression, anxiety, and certain forms of insomnia. It works by increasing levels of serotonin and norepinephrine in the brain, helping to improve mood and relieve tension. In lower doses, it is also effective for treating sleep disorders and chronic itching (pruritus). It is prescribed based on the condition, with the dose tailored to the individual.

Dosage and direction
For depression and anxiety, the typical starting dose ranges from 25 mg to 75 mg per day, taken once daily or divided into smaller doses. For insomnia, lower doses (3–6 mg) are often prescribed and taken 30 minutes before bedtime. Capsules or tablets should be swallowed whole with water. Always follow your doctor's instructions closely, and do not stop the medication abruptly.

Precautions
Before using doxepin, inform your doctor if you have a history of glaucoma, urinary retention, heart disease, or bipolar disorder. This medication can cause drowsiness, dry mouth, or dizziness, so avoid alcohol and be cautious when driving. In elderly patients, the risk of confusion or falls may be higher. Regular follow-ups are important, especially during the early stages of treatment.

Contraindications
Doxepin is contraindicated in individuals with untreated narrow-angle glaucoma, urinary retention, or known allergy to tricyclic antidepressants. It should not be used with monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping them. Caution is required in those with severe liver disease or recent heart attack. Not recommended during pregnancy unless clearly needed.

Possible side effect
Common side effects include dry mouth, drowsiness, constipation, and blurred vision. Some users may experience weight gain, urinary hesitation, or mood swings. Serious but rare effects include seizures, heart rhythm disturbances, or suicidal thoughts, especially in young adults. Notify your doctor if side effects are persistent or severe.

Drug interaction
Doxepin may interact with CNS depressants, alcohol, antihistamines, MAO inhibitors, and other antidepressants. Combining it with drugs that prolong the QT interval may increase the risk of heart rhythm problems. It may also interact with medications that affect liver enzymes, such as cimetidine. Always inform your doctor of all medications and supplements you’re using.

Missed dose
If you miss a dose, take it as soon as you remember, unless it’s close to the time of your next dose. Do not take two doses at once. If you are taking it for sleep, skip the missed dose and continue with your next scheduled one. Establishing a routine helps ensure effectiveness.

Overdose
Symptoms of overdose may include extreme drowsiness, confusion, seizures, irregular heartbeat, or even coma. Overdose is a medical emergency and requires immediate treatment. Supportive care in a hospital may be necessary. Do not exceed the prescribed dose under any circumstance.

Storage
Store doxepin at room temperature (20°C to 25°C / 68°F to 77°F), away from light and moisture. Keep in a tightly closed container, out of reach of children and pets. Do not use expired medication. Properly discard unused tablets as advised by your pharmacist.
